To be eligible for Oklahoma tax exempt vehicle registration, an organization and vehicle must qualify under an Oklahoma
statutory exemption that applies specifically to vehicle registration.
By completing this affidavit, you are acknowledging your full knowledge and understanding of the specific
Oklahoma statutory provision under which you are requesting this tax exempt registration.

Section II: Tax Exempt Vehicle Sign Display Requirements
(Note: Organizations devoted exclusively to youth programs pursuant to OS Title 47 Section 1135.1 (A) (2) (B) are
exempted from the following sign display requirements.)
Oklahoma law requires the name or symbol of a tax exempt organization to be displayed on the outside of the vehicle,
unless specifically restricted by federal or state law or state agency rule and regulation.
I hereby certify the name or symbol of my tax exempt organization is prominently displayed on each side of the
above listed vehicle in letters not less than two (2) inches high and two (2) inches wide and is of such shape and
color as to be readily legible during daylight hours from a distance of fifty (50) feet while the vehicle is not in motion.
I hereby certify that no signs or symbols are displayed on the above listed vehicle due to such display being
prohibited by federal or state law or by state agency rules and regulation. A copy of such federal or state law or
state agency rule and regulation must be submitted and attached to this affidavit.
