5. PORTFOLIO WEBSITE. The URL for the website must be active/live and accessible to

instructors on the due date and for the week following. Must be organized and easy to

navigate. All hyperlinks and documents must be accessible. Documents should be in Word

or PDF files. All the materials noted on this evaluation form must be posted on the website.

6. FINAL GROUP REPORT. Must be a minimum of two full typed pages. Should detail

what the project actually did, how it was implemented, what was learned, what was

accomplished, the results of the project, and the results of the measurement or assessment.

Your project team name, and the page number should appear in the header. A title “Final

Group Report” should be centered on the top line. (____/25 points, based on quality)

7. INDIVIDUAL WEEKLY JOURNALS. Must have journal entries each week. Should

reflect on group experiences and what lessons you learned from these experiences, while

incorporating concepts discussed during lectures and in the textbook. Scores can differ for

each group member. (____/10 points)

8. FINAL INDIVIDUAL REFLECTION PAPER. Must be a minimum of two full typed

pages. Should evaluate the entire project and group experience, incorporating concepts

from the textbook. Your first and last name, and the page number should appear in the

header. A title “Individual Reflection Paper” should be centered on the top line. Scores

will differ for each group member. (____/15 points, based on quality)
