Athletic Permission Form
It is the goal of the Monsignor Bonner & Archbishop Prendergast Catholic High School Athletic Program
to provide a safe and supportive environment for all student athletes. We believe athletes need to
develop skills that will teach good sportsmanship, self-discipline, and relationship skills. Toward that
end, coaches, students, and parents should be aware of school and county policies and procedures that
enhance these goals.
Please read carefully the conditions of participation. Please complete and sign Section IV and return this
form to the appropriate coach or the Athletic Director prior to your student’s taking part in any sport-
related activity. We look forward to your child’s active participation in our athletic program and
anticipate an excellent season. All actions of our students, athletes or not, are governed by our student
handbook. A copy of this handbook is available online.
Section I - Guidelines for Participants
For students to be eligible to participate in the Bonner & Prendie Athletic Program they must have on
file at their high school proof of meeting the following requirements. Requirements are to be met prior
to the first practice for any sport.
A. A completed Medical Information & Liability Release form
B. A completed PIAA Pre-Participation Physical Examination Form along with a physical

examination by a physician/authorized health care provider after May 31
of the preceding
school year.
C. Parental permission as provided below.
D. A paid Athletic Fee (due prior to start of the regular season as per the School Handbook)
Section II - Other Eligibility Requirements
These requirements must be verified as having been met prior to the first competition for any sport.
A. Academic Eligibility
A student must comply with the Academic Eligibility as outlined in the student handbook: “Students are
encouraged to participate in athletics and activities. However, their first responsibility is in the
classroom. They must meet academic standards in order to be eligible.
Students who fail to receive a passing grade in two or more subjects at the end of any quarter are on
Academic Probation and are ineligible to participate in sports or activities during the next marking
period. Specifically, students who receive two failures at the end of the school year may try out for
sports in the fall but are ineligible for the first two games of the season.
Students who are on Academic Probation are ineligible to participate in any sport or activity during the
first ten (10) school days of the next marking period. This is a total ban from all practices, games,
performances, work sessions, meetings, and any club activities.
After ten (10) school days, students may apply in writing for Probationary Reinstatement to the
Assistant Principal for Academic Affairs. If the student is passing all subjects and not missing
assignments, not just the ones that were failed the previous quarter, he or she may practice and attend
meetings but may not play in games, perform in the show or stage crew or attend any club activities that
