*St. Joseph’s Gingerbread Bazaar continues to be very popular. We thank our vendors for making it

ALL

such a success. In 2014,

craft booths were filled before August 1 with a waiting list.

We take pride in the fact that our Bazaar features

hand made items

and we want our Bazaar to

continue with the hand made items.

Vendors

must submit photographs of their crafts and a completed Wisconsin Tax Form when

submitting the registration form. Vendors who sell items that are NOT handmade will not be

approved as vendors next year, even if the vendor(s) has been a vendor in the craft show for several

years up to and including 2014.

St. Joseph’s Council of Catholic Women, as the “operator” of a temporary event is required to report

certain information to all sellers at the event, including those whose sales may be exempt from sales

tax (see attachment for Information). As the “operator”, we are responsible for providing seller

information to the Department of Revenue.

Please complete the attached form and return along with your registration form PRIOR to the date of

the Gingerbread Bazaar and Craft Sale. Completion of the Wisconsin Temporary Event Operator and

Seller Information – Section C of the attached form – will be required to participate in the

Gingerbread Bazaar.

ALL

participants must complete the tax form as St. Joseph’s CCW required to submit the information

to the State within sevens days of the event. If you have questions about the requirement to hold a

Wisconsin seller’s permit, you should contact the Wisconsin Department of Revenue at (608) 266-

2776 or send an e-mail to: sales10@dor.state.wi.us.
