The purpose of this evaluation is to help you reflect

upon your internship experience, identify areas of your personal and academic growth,

and trace your career path—whether it is related to the field of your placement or not.

The department appreciates your feedback and responses as they will be helpful not

only to yourself but also to students considering this internship organization in the

future. This evaluation form will be shared with your former internship supervisor,

but only after the placement is over. In other words, your responses will in no way

jeopardize the evaluation of your internship performance and the final grade of your

course. Please respond honestly and responsibly to the 12 questions below, and we are

open to opinions, suggestions, and constructive criticism.
