Registration Guidelines - Rockford Public Schools

BEFORE YOU WAIT IN LINE…
Is your child already assigned to a Rockford Public School? If the answer is NO,
you must first enroll your child. Please
proceed to his or her middle or high school. If the child is an elementary student, go to the Student Assignment Center, 2000 Christina St.
If a language other than English is spoken at home -- or if your child speaks a language other than English -- please go to the Student Assignment
Center, 2000 Christina St., to begin the enrollment process. That applies regardless of your child’s grade.
To enroll, please bring with you:
2 proofs of address (required – see below)
a certified copy of your child’s birth certificate (required within 30 days of enrollment)
an Illinois State Transfer Form (“Good Standing Form”), if transferring from another school in Illinois
a copy of your child’s Individualized Education Plan (IEP) if he/she requires Special Education Services
Have you moved from the last address Rockford Public Schools has on file for you? If the answer is YES,
you
will need to bring 2 proofs of the new address and fill out Change of Address paperwork before you can register today.
We must have proof of all required shots and physicals on file for your child to start school August 25th.
Has your child’s guardianship changed
If you answer YES,

The following proofs of address are acceptable when enrolling or changing addresses:

Mortgage Closing Papers
Most recent property tax bill and proof of tax payment
Signed and dated lease with cancelled bank statement or proof of last
Current Department of Human Services medical card
month’s payment (must be within last 30 days)
“Affidavit of Residence” Form when the person seeking to enroll a
Utility Bills (issued within last 30 days)
student is living with a District resident and the resident presents two of
the proofs of address listed along with a valid ID. Copy is then sent to
District Legal Dept. for review.
Mail received at current address (only ONE piece of mail will be
accepted)
