BEFORE SIGNING

This is a legal document which, if signed, will have the effect of rendering you and your child

unable to bring any type of action against the Hopkinton Public Schools, the School Committee

for the Hopkinton Public Schools, the Town of Hopkinton, or any of its or their agents, because

of any harm you or your minor child may suffer as a result of his/her transportation to any

school bus stop other than his/her regular, residential bus stop of the Hopkinton Public Schools.

This agreement cannot be applied to bus transportation that conflicts with the district day to

day bus switching policy.

HOPKINTON PUBLIC SCHOOLS

CONSENT AGREEMENT AND WAIVER OF LIABILITY

1. I/We, ____________________________, as parent(s) or guardian(s) of

__________________________ , a minor child (hereinafter, the “Student”), hereby

acknowledge that the Hopkinton Public Schools provides bus transportation for the

Student between the Hopkinton Public Schools and the Student’s regularly scheduled

residential-area bus stop (hereinafter, the “Regular Residential-Area Bus Stop”) in

connection with his/her attendance at the Hopkinton Public Schools. I/We have

requested, and may from time to time throughout the school year, request that the

Hopkinton Public Schools permit the Student to ride school bus routes and to be

discharged from the school bus at bus stops other than the Student’s Regular Residential-

Area Bus Stop (such other bus routes and stops are hereinafter referred to as the

“Alternate Bus Stops” and/or “Alternate Bus Routes”). I/We, as parent(s) or guardian(s)

of the Student hereby give our consent to the Hopkinton Public Schools permitting the

Student to ride Alternate Bus Routes and to discharge the Student from the school bus at

Alternate Bus Stops. I/We further acknowledge and agree that the Student’s use of the

Alternate Bus Routes and the Alternate Bus Stops is completely voluntary on his/her part

and on my/our part, as parent(s) or guardian(s) of the Student. Notwithstanding the

foregoing, I/We acknowledge and agree that, as the parent(s) or guardian(s) of the

Student, on those occasions when I/we wish to have the Student transported from the

Hopkinton Public Schools to locations other than the Student’s Regular Residential-Area

Bus Stop, I/we have primary responsibility for providing such transportation. As such,

I/We acknowledge and agree that the Hopkinton Public Schools shall have the right at

any time and from time to time to refuse to transport the Student along such Alternate

Bus Routes and to such Alternate Bus Stops for any reason or for no reason and, as such,

may at any time require the Student to ride his/her regular residential bus route and to be

discharged at his/her Regular Residential-Area Bus Stop. I/We acknowledge and agree

that it is the Student’s responsibility to board the appropriate bus at the designated time

and that the Hopkinton Public Schools is not responsible to see or insure that the Student

boards the particular bus with the Alternate Bus Route and Alternate Bus Stop which

I/we intend the Student to board.

2. For good and valuable consideration, including but not limited to the Student being

allowed to use and/or participate in the Alternate Bus Routes and Alternate Bus Stops,

the receipt and sufficiency of which consideration I/we hereby acknowledge, I/we do
