SECONDARY SCHOOL STUDENT COMPLAINT FORM – HARASSMENT
If in your opinion, the behavior of a School District student or employee causes you to
feel that you are being harassed, you are urged to report that conduct to your principal, or another
school administrator, or a professional employee at your school. All reports received will be
referred to the principal. The principal will notify the Assistant Superintendent, Human
Resources and Operations of your complaint and an investigation will be conducted. You will be
notified of the results of the investigation.
If you prefer to file a written complaint, rather than verbally reporting your concern,
please complete the attached form and return it to your principal.
You are urged to file your complaint as soon as possible, following the incident giving
rise to your concern. The assistance and support of your parent(s)/guardian(s) is welcomed
during all phases of the processing of your complaint, including the initial reporting of that
complaint.
