2016 Summer Camp Field Trip Form

Purpose: To enhance the day camp curriculum and camp experience.

Departure: All trips will leave at approximately 9:30am from Plex, unless otherwise specified to parents

in advance. Please have your child here by 9:00am on field trip days.

Arrival: Most trips will arrive back to Plex by 1pm; some trips may arrive back later. Parents will be

informed if a later pick-up is required.

Chaperones: Day Camp Administrative Staff and counselors

Please have a parent or guardians sign the permission slip below. NO STUDENT WILL BE ALLOWED

TO GO ON THE FIELD TRIP UNLESS THEY HAVE RETUNED A SIGNED PERMISSION SLIP.

My son/daughter____________________________ (name) has permission to accompany the Plex day

camp staff on ALL field trips for Plex Summer Camp 2016. I understand that they will travel on a Plex bus

by a licensed CDL driver. All campers will be subject to the day camp disciplinary policies and

procedures.
