ART EXHIBITION LIABILITY RELEASE FORM

This AGREEMENT (“Agreement”) is between

_______________________________________________________________ (“User”),

whose mailing address is

_______________________________________________________________________

and Moorings Presbyterian Church, Inc. (“Church”), who’s mailing address is 791

Harbour Drive, Naples, Florida 34103.

Church has agreed to sponsor an Art Exhibition to display all forms of visual art of

members and friends of Church and further agrees to permit User to display art on the

Church premises (“Permitted Use”);

As a material inducement for Church to allow the Permitted Use, User agrees:

The Exhibition is to be held April 13, 2014 to April 20, 2014.

User shall deliver art to be exhibited no earlier than April 10, 2014 and will remove all

exhibited art no later than April 21, 2014.

User agrees that it will not use the premises for any unlawful purpose and will obey all

laws, rules and regulations while using the above described facilities.

User agrees that the Church has no responsibility to provide security or insurance to

cover User’s property.

User understands that Church does not know the value of User’s property and cannot

assure that there is sufficient insurance coverage for the property. Therefore, User

promises and warrants that it carries insurance sufficient to cover any damage, loss or

theft of User’s property.

Church and User agree that any disputes arising under this Agreement will be resolved

via a mutually acceptable alternative dispute resolution process.
