VOLUNTEER RELEASE

On this _______ day of __________________, 20___, intending to be legally

bound hereby, the undersigned volunteer agrees and does hereby release from liability

and to indemnify and hold harmless Springs Ensemble Theatre (hereinafter “SET”), and

all officers, directors, employees, agents and volunteers representing or related to SET,

acting officially or otherwise, from any and all claims, demands, actions or causes of

action which in any way arise from the undersigned’s participation in SET’s productions.

This release is for any and all liability for personal injuries (including death) and property

losses or damage occasioned by, or in connection with, any activity. The undersigned

further agrees to abide by all the rules and regulations promulgated by SET, and any of

its employees or agents, and/or its affiliate groups and vendors throughout

undersigned’s participation. The undersigned also specifically allows SET to photograph,

record, film or videotape undersigned in the course of undersigned’s participation in

SET’s productions. Undersigned also specifically allows SET to collect undersigned’s

personal contact information including, but not limited to, name, phone number, and

email address, and to provide undersigned’s contact information and any photographs,

recordings, films or videos taken by SET to SET’s media and marketing contacts for the

purposes of promoting SET and its productions.
