Leave of Absence Packet
All types of special leave require supporting
documentation.
The Leave of Absence request
will not be considered without this documentation
.
Once this request has been submitted to
HR, a Pre-Leave Conference (a meeting to discuss the need for the Leave) will be scheduled
with Sharon Hunt.
Employees are responsible for maintaining their Health/Dental/Prescription Benefit co
payments while on leave without
pay.
If an employee fails to pay the necessary co-payments
,
his/her benefits will be terminated
.
An employee whose benefits are terminated is eligible for
COBRA benefits and the employee will be so notified
.
When the employee returns to
work,
their benefits can be reinstated by completing Health Benefits and Dental Applications and
forwarding them to the Office of Human Resources by the second day of the employee's
return. Employees may also opt to reduce their level of coverage or waive coverage while on
leave of absence.
Employees on leave must notify the Office of Human Resources at least two weeks prior to the
end of their leave whether they are going to return to work (with the necessary
documentation),
resign in good standing or file for retirement. An employee who fails to return on their
designated date without notifying the Office of Human Resources of their intention, as
described herein, may be terminated for abandonment of position after five (5) days of the date
of designated return.
