Dear Doctor:
Your patient has chosen to fly with Cathay Pacific Airways or on an interline ticket associated
with a Cathay Pacific Airways flight.
At the time of the booking inquiry, the information provided to our Reservations office has
prompted Cathay Pacific to ask that your patient complete our MEDA (Passenger Medical
Clearance) form. Part 1 of the form should be completed by the passenger and Part 2 should be
fully completed by the treating doctor in English. Once the MEDA form has been completed, it
should be returned to Cathay Pacific’s local Reservations office.
Any costs associated with this assessment and completion of the form is at your patient’s
expense.
It is important that Cathay Pacific receives the completed MEDA form no later than 48 hours
prior to your patient’s scheduled departure date.
In an aircraft cabin your patient will be exposed to the effects of air pressure changes, reduced
oxygenation and humidity, turbulence and being in a confined area. Given that passengers are
exposed to these special conditions during air travel, it is important that you consider your
patient’s medical status carefully before you complete the MEDA form. Additional information is
