If your family’s 2014 income is significantly less than your family’s 2013 income due to one of

the circumstances described below, you qualify for a special circumstance review. If your

situation is not reflected in any of the conditions outlined below and you feel you may qualify for

a special circumstance review, please contact the Office of Student Financial Services.

In order to have your special circumstance request reviewed you must complete this form and

submit supporting documentation. Acceptable documentation is outlined below. In addition, you

must complete the regular verification process which includes a 2014-15 verification worksheet,

2013 IRS Tax Transcript if the IRS Data Retrieval was not used, and W2 forms if you had

income earned from work and did not file taxes.

__ I am independent student and my income and/or my spouse’s income will be substantially

less in 2014 then 2013

__ I am a dependent student and my parent’s income will be substantially less in 2014 than 2013

Please check the reason that best describes your situation and supply all the required

documentation listed.

__ Unemployment: You, your spouse, or your parent who worked in 2013 has been unemployed

for at least 4 weeks in 2014 and will earn less money in 2014 than 2013.

Please Supply:

 A typed statement that includes dates and explains the changes in your employment

history from January 1, 2013 till present

 Copy of final pay stub containing year to date earnings

 Documentation of any benefits received in 2014 (unemployment, severance etc.)

 Termination letter (if received)

 2013 verification documents

__ Separation/Divorce: You or your parents divorced after the date you filed the FAFSA.

Please Supply:

 Proof of divorce or separate addresses

 Letter of explanation giving dates and details as to disposition of assets etc.
