The California Student Aid Commission requires the Financial Aid Office to obtain
documentation that verifies you have met the following eligibility requirements before you
may receive your Transfer Entitlement Cal Grant Award. Your Cal Grant is on HOLD by
CSAC for AB840 verification.

Graduate from a California high school (unless due to military orders that required that you or your parent or
guardian to be out of state at the time of your graduation) or have completed a high school graduation equivalency
(including GED or the California High School Proficiency Exam) on or after June 30, 2000.
Be a California resident at the time of your high school graduation, or its equivalent.
Verified Community College GPA of at least 2.4
Attended and completed at least one term at a Calif. Community College and be transferring to a qualifying Calif.
institution that offers a bachelor’s degree for 2014-2015.

Read the following carefully then submit this completed form and all requested documents to the Financial Aid Office as
soon as possible. Failure to return this information could adversely affect your ability to receive a Cal Grant award.
1. Submit a copy of your California high school transcript or high school diploma that shows your graduation date, or
proof of equivalency AND one of the following.
a. A copy of your parent’s (or legal guardian’s) California state tax return for the tax year immediately prior
to your high school graduation or its equivalent (for example, a 2007 California state tax return if you
graduated in 2008)
b. If your parent (or legal guardian) did not file a California state tax return for the year requested, your
parent (or legal guardian) must provide one of the following documents for the calendar year immediately
prior to your high school graduation or its equivalent.
California lease or rental agreement
California auto registration
California utility bill
California bank account record
California voter registration
OR - For military personnel:
2. Submit a copy of your parent’s military records verifying relocation orders from California dated prior to your high
school graduation or its equivalent AND at least one of the documents listed above for a period that predates the
relocation orders.
