Parents: Please write the date, book title, and author on the form below after you have your child has read it or
you have read it with your child. Beginning readers will develop more self-confidence in their reading skills, if
they are encouraged to reread a book. Please aim for reading at least 20 minutes a day and record on the log at
least 25 reading times (not 25 books) over the course of the summer. A book report is required for at least four
books read. Both the log and the forms must be turned in at the beginning of the school year.
