CADI Case Manager/Service Coordinator

Contracted Case Management – Hennepin County
Qualifications:
One year of experience in the delivery of social services to adults with
mental illness or brain injury
Experience working with adults with mental health diagnoses,
homelessness, or complicated medical conditions
Ability to work independently and in crisis situations
BA/BS in social work, nursing, psychology, sociology or a closely related
field
Pass DHS background check, valid driver’s license and clean driving
record
Good written and oral communication skills
Reliable transportation
Responsibilities:
Comply with case management rule 185
Development of a community support plan
Informing the person or legal guardian of service options
Identification of potential providers
Assist the person to access services, develop service agreements and
documentation through case notes
Evaluation and monitoring of the services identified in the plan
Comply with MN Health Care Programs in relation to the delivery of waiver
services including having LTC & PCA assessments completed and
individual plans in place.
Salary:
BA/BS $18.32-19.05/hour DOE
MA/LSW starting at $19.43/hour

40 hours/week
