This statement is for the purposes of providing information to support fitness for human consumption, product
traceability and suitability, in accordance with requirements of the Food Act 1981 and the Animal Products Act
1999.

Only veterinary medicines or agricultural compounds permitted for use in
beehives or beekeeping equipment have been used, and they have been used in
accordance with any label or approval conditions (also tick “Yes” if no veterinary
medicines or agricultural compounds have been used).*

The bee products and beekeeping equipment have been stored and transported
under conditions which minimise exposure to contamination.*
(d)
All apiaries are operated in compliance with the American Foul Brood Pest
Management Strategy. *

why this product should be considered suitable for human consumption. If you have answered “No” to (d) above the bee products are not eligible
for export to the European Union for human consumption. Note Para di-chloro benzene (PDB) is not permitted for use in beehives or beekeeping
equipment.

The details will be held by the RMP operator identified in this form; and

Bee Products as notified under section 60 of the Animal Products Act 1999. The provision of this information is necessary in
order to provide information to support the fitness for human consumption, product traceability and suitability of bee products for

Failure to provide the requested information will result in bee products being ineligible for export with official assurances; and
