Checklist For Fuel Distributors License Applicants
The following information is being provided to ensure that your application is not missing
any information. We strongly recommend that you use this document to aid you in
completing the application packet. You may download all forms and access RevenueOnline
from our Web site:
Application: All requested information provided including all boxes marked "Yes" or "No." N/A if
something does not apply. Application is signed by owner, corporate officer or LLC member. If applying for
an importer and/or exporter license, please enclose photocopies of your fuel distributor license(s) for the
other jurisdictions.
Fuel Distributors Bond: Equal to three (3) times the monthly tax liability, minimum bond for both gasoline
and/or special fuel being $25,000.00, and maximum bond for both gasoline and/or special fuel being
$200,000.00. If the monthly tax liability is unknown, please estimate this amount for all fuel types involved.
Please use form DR 7065 to submit the Fuel Distributors Bond.
Application for EFT Payments: Effective July 1, 2005 all licensed fuel distributors are required to remit
their tax payments via EFT. The EFT application (DR 5785) must be completed and included with the
application. An EFT account may also be set up on the Department's website on RevenueOnline. You may
access online setup at
Financial Statement: Must include a report of assets, liabilities, net worth, income and expenses. The
documents must be signed, certified and notarized by the owner or authorized representative of the
company. Financial statements with an opinion from a third party (independent auditor) may be accepted in
lieu of a notary.
Trading Partner Agreement: Colorado law requires fuel excise taxes be filed via electronic filing
with the Colorado Fuel Tracking System. Please complete the online form and include the form with
your application. Once the account has been approved and a license number issued, the Colorado Fuel
Tracking System personnel will contact you with further instructions. You may also reach them at 1-877-

Fees: Enclose check made payable to the Department of Revenue, $10.00 processing fee per application
and a $5.00 fee for additional Colorado bulk storage locations (not including retail outlets).
Failure To Submit A Complete Packet Will Result In The Rejection Of The Application.
