3. As provided in Chapter 201, Subchapter C, Section 201.027of the Texas Unemployment

Compensation Act, the above named domestic employer hereby makes application to

withdraw their Election By a Domestic Employer to Report Quarterly Wages and Pay

Taxes on an Annual Basis. It is understood and agreed that there must be a minimum of

two (2) calendar years and a timely application submitted before the method of filing

quarterly reports may again be changed.

3. The effective date of this withdrawal is January,
