Federal Form 4835, Farm Rental Income and Expenses, Schedule F (Form 1040), Profit or Loss From Farming, or Schedule C (Form
1040). Applicants who are not required to file federal Form 1040 must also submit the same information as would be reported on
Schedule C or Schedule F even though they are not required to file those schedules for federal tax purposes.
