A REQUIRED DOCUMENTS CHECKLIST FOR YOU

Take the following documents with you when applying for a driver license or temporary

instruction permit at any Department of Revenue contract license office:

1) Name, Date of Birth, and Place of Birth

U.S. Citizen — Provide one of the following:

• U.S. Birth Certificate - certified with embossed, stamped or raised seal issued

by a state or local government (Hospital Certificates are not acceptable);

• U.S. Passport (valid or expired);

• U.S. Certificate of Citizenship/Naturalization/Birth Abroad; or

• U.S. Military Identification Card or Discharge Papers accompanied by a copy

of U.S. Birth Certificate issued by a state or local government.

Non-U.S. Citizen — Must present appropriate immigration documents indicating

current status. (Refer to the Department of Revenue web site at

for a complete listing of required documents.)

2) Social Security Number — U.S. Citizen or Non-U.S. Citizen must provide:

• Social Security Number (verbally or by presenting card); or

• A letter from the Social Security Administration (SSA) regarding the status of

your Social Security Number if a number has not been assigned.

3) Name Change — Present one of the documents below showing your current name:

• Certified Marriage Certificate;

• Certified Divorce Decree;

• Certified Court Order;

• Certified Adoption Papers;

• Amended Birth Certificate;

• U.S. Passport (valid or expired); or

• Social Security Card/Medicare Card.

4) Missouri Residential Address — Provide one of the following:

• Voter Registration Card;

• Utility Bill;

• Bank Statement;

• Government Check;

• Pay Check;

• Property Tax Receipt;

• Housing Rental Contract; or

• Mortgage Document.
