A bus, truck or truck tractor registered for a gross weight of more than 12,000 pounds, which is operated wholly within the corporate limits of a city or village or
within a radius of 25 miles beyond the corporate limits, shall be classified as a local truck except that in no event shall such vehicles operated as contract or
common carriers outside a radius of three miles beyond the corporate limits of the city or village in which such vehicles were based when registered and
licensed be considered local trucks or truck tractors.

I hereby by declare the vehicle listed in this affidavit shall not be operated for more than a total of six thousand (6,000) miles during the registration period/year
(Jan. 1 to Dec. 31). If this vehicle is operated for more than the 6,000 miles during the registration period/year, I will return to the county’s motor vehicle office
and make application for regular registration and pay the additional fee difference due for the regular registration. I further declare the vehicle’s odometer will
be maintained in constant working order and the required mileage/trip log will be maintained at all times. I understand and agree if the 6,000 limit is exceeded
and I did not apply for regular registration as required, I will not be eligible to renew the 6,000 mile registration the following year and will be required to obtain
regular registration at renewal time.

K.A.R. 92-51-32 Six thousand (6,000) mile registration requirements. The speedometer or odometer which records the number of miles traveled by a
vehicle having a 6,000-mile registration shall be kept in working condition at all times during the registration period. The highway patrol, motor carrier inspection
bureau, or any law enforcement officer shall have authority to inspect the readings of the speedometer or odometer and to inspect the vehicle to ensure that the
speedometer or odometer is properly connected and in good working condition. The operator of this vehicle shall keep at all times in the vehicle a log reciting
the date, the towns traveled in, and the distance operated on each trip. Anyone securing a 6,000-mile registration and not having the speedometer or odometer
connected and in good working condition or not having the log in the vehicle shall have the vehicle registration suspended and the registration shall not be
reinstated until the difference between the 6,000-mile and the regular registration fee on the vehicle is paid. (Authorized by K.S.A. 74-2011, 8-134; and 8-143)
Trip/Mileage Log Book Requirement: The operator of this vehicle shall maintain at all times in the vehicle a log book reciting the date, to and from locations
and distance traveled for each trip. Any one operating a vehicle registered with a 6,000 mile registration shall assure the vehicle’s odometer is connected and is
accurately recording the actual miles the vehicle is traveling. Failure to maintain the log book and/or odometer as set forth herein shall have their 6,000 mile
registration suspended. The vehicle will not be registered again until the difference in fees between the 6,000 mile registration and regular registration has been
paid. The owner of the vehicle will not be eligible for 6,000 mile registration in the future.
