Registration Details
 Registration Forms must be received by Eagle Eyrie no later than June 30, 2017.
 Eagle Eyrie will bill separately for lodging and meals once accommodations have been assigned.
 All registration fees are payable to “Eagle Eyrie Baptist Conference Center.”
 Send all money (no cash) and one copy of the Registration Form to Eagle Eyrie.
 All participants are required to have both a Medical Information Form and a General Release & Authorization
Form on file. Please print the Medical Information Form and General Release & Authorization Form on the
same sheet (front and back). One sheet works best for storing in each participant’s nametag lanyard.
 Bring 4 copies of each participant’s completed Medical Information Form and 4 copies of each participant’s
completed and signed General Release & Authorization Form to MC2. Do not mail these forms.

Faculty Info: Individuals who have been enlisted as Faculty members will receive a special code from their
respective leaders. Youth Faculty members will receive one from Dawn Fisher, Children’s Faculty members
will receive one from Lisa Dockery, and Preschool Faculty members will receive one from Rebecca Ann
Woolard.
