8If you are able to document that you have satisfied the requirements outlined, your request will be approved
and your CASAC or CPP/CPS certificate will be reissued with the Specialty designated. The Specialty Desig-
nation will not affect the effective or expiration date of your current credential and will be renewable as part of
the standard renewal process during your next credentialed period (see requirements for renewal presented
below).
If your Request Form is determined to be incomplete, you will be mailed a Review Summary identifying any
deficiencies. Identified deficiencies must be satisfactorily addressed prior to the expiration date of your cre-
dential. If you are unable to address the deficiencies by the expiration of your credential, your request will not
be considered until your credential is renewed.
Complete the attached form and submit it, along with all required documentation, with a one-time non-
refundable $25 Specialty Designation Fee in the form of a check or money order made payable to NYS
OASAS. Personal checks will be accepted, however, please be aware that submission of a certified
check or money order will expedite the processing of your Request Form. Request Forms must be

RENEWAL REQUIREMENTS
In order to renew your Specialty Designation, you will be required to document completion of 10 clock hours of
continuing professional education specific to problem gambling in addition to the continuing professional
education required to renew your CASAC or CPP/CPS certificate.
The fee associated with the renewal of your CASAC or CPP/CPS will not increase as a result of holding a
Specialty Designation.
The Specialty Designation will not affect the effective or expiration date of your current credential.
