SECOND CHANCE PROBATION ORDER
Conditions of Probation pursuant to Illinois Compiled Statutes Chapter 730, Section 5/5-6-3.4
On this day, Defendant herein, having admitted to the facts which form a basis for the offense of:

, and the Court being duly advised in the
premises: That the Court being of the opinion that neither a term of imprisonment nor of a formal probation is appropriate being
that the Defendant does not have any prior offense under 730 ILCS 5/5-6-3.4(a) and the Defendant is not pleading to and has not
been convicted of a violent offense under Section (a- I).
It is hereby ordered that Defendant be admitted to a term of probation of 24 months and that as a condition of said probation,
the Defendant:
1. Shall not violate any criminal statute of this State or any other jurisdiction;
Is not ordered to report to the Will County Probation Dept.

Is ordered to report to the Will County Probation Dept.
3. If ordered to report to the Will County Probation Department, the Defendant shall report this date and at such other dates and
times as directed; and shall permit a probation officer to visit the defendant's home or elsewhere as necessary to discharge the
probation officer's duties; and shall pay a probation fee of
a month;
4. Shall refrain from possessing a firearm or other dangerous weapon;
5. Shall refrain from having in his or her body the presence of any illicit drug prohibited by the Methamphetamine Control and
Community Protection Act, the Cannabis Control Act, or the Illinois Controlled Substances Act, unless prescribed by a
physician, and submit samples of his or her blood or urine or both for tests to determine the presence of any illicit drug;
6. Shall submit to periodic drug testing at a time and in a manner as ordered by the Court, but no less than 3 times during the
period of probation, with the cost of the testing to be paid by the Defendant;
7. Shall notify the State's Attorney's Office and the Clerk of the Circuit Court of any change of address within 72 hours;
8. Shall perform no less than 30 hours of community service work;
