Disclosure Consent Declaration
If you choose to use TeleFile to file both your state return and your federal return at
the same time in one telephone call, you must first consent to share your federal
information with the state. The IRS may then share with the state all of the federal
information needed for the filing of your state return. The state may use this infor-
mation in the same way it uses information provided on a paper return.
The telephone messages will tell you how to do the consent during the call using
the “disclosure consent declaration.” You will be asked to use the IRS Customer
Service Number in the IRS tax package to authorize the sharing of the information.
Once you have agreed to share the information, you may not cancel the agreement.
How Does Federal/State TeleFile Work?
File your federal return and Kentucky return by phone—at the same time—using the IRS TeleFile
toll-free number.
Complete the top half of the federal tax record found in the federal TeleFile tax package you
received from IRS.
Complete Line A of the Kentucky tax record.
Complete Line B of the Kentucky tax record if you choose to deposit your refund directly in your
bank or credit union account.
Call IRS TeleFile toll free, 24 hours a day.
(The telephone number can be found in the federal Telefile tax package.)
Keep the federal and Kentucky tax records for your records.
(Do not mail tax records to IRS or the Kentucky Revenue Cabinet.)
What Happens During the Phone Call?
When you call TeleFile, you will . . .
File your federal tax return. You will provide information needed to complete your federal tax
return. TeleFile will compute your federal taxes for you and give you a confirmation number to
guarantee that IRS has received your federal tax information.
Agree to share your federal tax information with the Kentucky Revenue Cabinet. This is a consent
to disclose your federal tax information with the Revenue Cabinet. This is needed to complete
your Kentucky return. The Revenue Cabinet will keep your return information confidential.
File your Kentucky return. You will only provide the additional information needed to complete
your Kentucky return—you will not need to provide information more than one time. TeleFile will
compute your Kentucky taxes for you and give you a confirmation number to confirm that you
submitted your Kentucky return.
IMPORTANT: Do not hang up before you receive your Kentucky confirmation number. If
you want to file your state return with TeleFile, you must do so during the same phone
call when you file your federal return. After filing your federal return, if you hang up
before you finish filing your state return, you cannot call back later to file your state
return with TeleFile. You will have to file a paper return, Form 740-EZ.
