Vessel & Captain Authorization Form

The Commercial Fisheries Entry Commission will issue CDQ crab permit cards for 2001 only to individuals

authorized as vessel captains by Bering Sea Crab CDQ groups. The manager or director of each CDQ group must

complete this form in advance of the 2001 CDQ Crab season, designating the vessel(s) and captains operating in

each CDQ fishery for which allocation has been received. Additional copies of this form may be submitted by fax if

additional captains are authorized at a later date. Please notify the commission immediately if a captain is no longer

authorized to fish for the designated CDQ group.
