APPLICATION & INSTRUCTIONS FOR

ASBESTOS BUSINESS LICENSE

CODE-620

Asbestos Business is a partnership, firm, association, corporation, or sole proprietorship that is engaged in

asbestos abatement.

Please read and follow these instructions carefully. A correctly completed application will expedite the issuance

of an Asbestos Business License.

A completed Miscellaneous application for license (BRA-4) is required. (attached)

A Corporation, Limited Liability Company or Limited Partnership doing business in the District of Co-

lumbia must be in good standing. For additional information, call (202) 442-4430.

A Certificate of Occupancy is required for a commercial place of business in the District of Columbia. For

additional information, please call (202) 442-4567 or visit the Permit Issuance Branch on the 2nd Floor,

Room #2300.

A Tax Registration Certificate is required from the Office of Tax and Revenue. You may obtain this

certificate from the Satellite Office, Room #1110, or call (202) 727-4829 to obtain additional informa-

tion.

A Certified Resident Agent Appointment Form is required for applicants who are non-residents of the

District of Columbia.

A self-certification is attached and must be submitted along with the application for lincensure. (attached)

The applicant shall submit a cover letter listing the names of the asbestos workers who will be employed by the

company. Any asbestos worker who arrives without this cover letter will be required to complete a tax registra-

tion application as an independent contractor.
