State law requires that before information may be obtained concerning unclaimed property that has been
reported, a person must show proper identification to establish an interest in the property.
To allow the Unclaimed Property Division to process your inquiry, please send a BLACK AND WHITE copy of your
drivers license and a copy of your social security card or proof of your social security number. Supplying Social
Security Numbers (SSN) is voluntary. However, failure to submit SSN information may result in denial of a claim. SSN’s
are used to match the claimant to the lost owner and to complete tax form 1099 for certain income property. By law, your
personal information is kept confidential by the Unclaimed Property Division. Authority OAC 735:80-7-2.

When your inquiry is received by the Unclaimed Property Division, you may be asked to provide additional
documentation to substantiate your claim. This is for your protection as well as ours. Our goal is to reunite unclaimed
property with the true lawful owner. There are no fees or charges connected with your inquiry or payment
of claim by the Treasurer.
Your inquiry must be in writing. The value and type of unclaimed property or the identification of the property
holder cannot be obtained by phone. Due to the high volume of inquiries, your claim will be processed as quickly as
possible.
