CMFT-2 Instructions
You must file Form CMFT-2, Multiple-Site Form, if you make retail sales of motor fuel from more than one place of business located within a
county that imposes a county motor fuel tax.
Column A
Write the total number of taxable gallons of motor fuel you sold at retail from each site within a county that imposes the County
Motor Fuel Tax.
Note: Do not include gallons deductible by law.
Column B
Multiply the number of gallons in Column A by the tax rate. Write that amount on the corresponding tax line in Column B.
Column Totals
Column A
Add the amounts you wrote in Column A. Write this amount on the line for the Column A total. If your Form CMFT-2 has more
than one page of sites, complete the column totals for each page. The combined Form CMFT-2 Column A totals must equal
Line 4 on Form CMFT-1.
Column B
Add the amounts you wrote in Column B. Write this amount on the line for the Column B total. If your Form CMFT-2 has more
than one page of sites, complete the column totals for each page. The combined Form CMFT-2 Column B totals must equal
Line 5 on Form CMFT-1.
Note:
When filling out this form, you must round to the nearest dollar by dropping amounts of less than 50 cents and increasing
amounts of 50 cents or more to the next higher dollar.
