This guest player permit is for use by the team borrowing the Minnesota Youth Soccer Association (MYSA) / US Youth Soccer (USYS) registered

guest player who has been given permission by his/her MYSA registered (lending) team coach to participate as a guest in a USYS out-of-state

sanctioned event. This guest player permit is valid only for the event and dates indicated herein.

Enter the required information as specified. Use a separate form for each guest player, activity, date, location, coach, team.

The guest player must be a currently registered MYSA player who has been released by his/her coach to play as a guest player with the borrowing team for the tournament

indicated. The guest player must use the player pass from his/her MYSA currently registered league team.
