GENERAL INSTRUCTIONS

This brand registration application must be filed with the Department of Business and Professional

Regulation. Please complete all information. Incomplete applications will be returned. All questions are

applicable and must be answered fully and truthfully.

You must provide an original and a copy of the application. All signatures must be original.

This form is to be used for Primary American Source of Supply licensure for vinous and distilled spirits,

brand/label registration and "FL/Florida" imprinting approval/exemption for malt beverages, and revised

labels. Indicate the type of application by checking the appropriate box(es) in Section 1 of the application.

The fiscal year is from July 1 through June 30.

APPLICATION REQUIREMENTS and INSTRUCTIONS ON COMPLETING THIS APPLICATION

Application Requirements

Corporate and Limited Partnership Registration

All corporations, domestic or foreign; general partnerships; limited liability corporations; and limited

partnerships are required to be registered with the Florida Secretary of State, Division of Corporations. If

you have not already registered, you will need to contact the Department of State at (850) 488-9000 for

further information.

Fees

The fee for each brand/label is as follows:

1. Distilled Spirits:

$30

2. Malt Beverages:

$30

3. Vinous:

$15

Payment should be made payable to the Department of Business and Professional Regulation. There is

no fee for a revised label application or to add additional size(s).

Confirmation of Approval

Each brand/label approved will be assigned a brand number, which will be entered in the "Brand Number"

field of the application by the Division. The duplicate application submitted will be returned to you as

your confirmation of licensure/registration.
