1. SIGNATURE:
CORPORATIONS - An application for reinstatement by a corporation forfeited for failure
to file a franchise tax return and/or pay state franchise taxes must be signed by an officer,
director or shareholder of the corporation at the time of forfeiture. All other applications
must be signed by an officer or director of the corporation.
LIMITED LIABILITY COMPANIES - An application for reinstatement by a limited
liability company forfeited for failure to file a franchise tax return and/or pay state
franchise taxes must be signed by a manager or member of the limited liability company at
the time of forfeiture. All other applications must be signed by a manager or member at the
time of reinstatement.
FEES: The fee for an application for reinstatement is $75 unless the entity is a nonprofit
corporation. The filing fee for the reinstatement of a nonprofit corporation is $5 unless the
corporation was forfeited for reason (b), in which case there is no fee.
