THINGS TO KNOW BEFORE STARTING YOUR APPLICATION
This form must be lodged to obtain the Registrar's prior approval for changes to be made to the following rules before a resolution
amending the rules is passed by the co-operative:
* Rules under section 60(2) of the Co-operatives National Law (NSW) relating to the Active Membership Provision as made by

designated instrument published in the NSW Government Gazette on 7 March 2014.
* Rules relating to the conversion of a non-distributing to a distributing co-operative under section 60(1)(b) of the Co-operatives
*
National Law (NSW).
* Rules of certain co-operatives formed to carry on as a club that restrict voting rights under schedule 1(5) of the Co-operatives
*
(Adoption of National Law) Act 2012.

* The application fee is $72.00
How to lodge and pay
* If lodging by email, email scanned form to registrylodgements@finance.nsw.gov.au.
* If lodging by post, post the completed form and payment to Registry Services, PO Box 22, Bathurst

NSW 2795.
* If lodging in person, form may be lodged and payment made at any Service NSW Centre.
* The application fee must be paid at the time of application. There is no GST payable. Payment can be made by
* cheque or money order made payable to `NSW Fair Trading'; or
* credit card by completing the payment details on last page. Please note that when paying by credit card a surcharge of 0.40%
* *
will be charged on the total amount due.
* Receipts will only be issued on request.
What happens when you lodge your application
* The application will be reviewed. You will be notified in writing if further information is required.
* If your application is approved, you will receive written notice of that approval.
* If your application is refused, you will receive written notification of the reasons.
* If any change occurs in the information you have provided in your application, you must notify NSW Fair Trading as soon as possible.
