Social Security Number Submission Form

Iowa Western will use your social security number to verify your identity for record‐

keeping purposes and to help match your academic information and other materials

with your student record. Although it is not required for admission, your social security

number is required to be considered for Financial Aid loans and grants.

In addition, Iowa Western is required to provide a Form 1098‐T to each student (this

information is also submitted to the IRS), which you will need in order to claim an

education credit on your taxes, if you are eligible for such a credit, and we must have

your social security number in order to do so.

Finally, Iowa Western has partnered with Higher One to offer all students enrolled in six

credit hours or more a new college refund card called the Reiver Card; however, we

must have your social security number on file before we can send your Reiver Card.
