Switch Kit
At CapEd we’re making it easy for you to maximize your money and your time. Through your
home PC, telephone, ATM’s or in the Credit Union, you will be able to take care of all your
financial needs at one time, and in less time. We’re making it simple for you to switch all your
accounts to CapEd and use us as your primary financial institution. Making the switch to CapEd
provides you with convenient and personalized financial service! We want your business!
Each form enclosed in this booklet will assist you in making the switch. Simply fill out the forms
enclosed and mail them to the appropriate entities. A checklist is also included to assist you with
closing your existing checking account.

Membership Application – Fill out the membership application and mail it or bring it into one of
our six branches and a MSR will be glad to help you.
Direct Deposit Form – If your employer offers Direct Deposit, simply fill out the form and take it
to your payroll department. For Social Security and Federal payments, please fill out the Direct
Deposit form #1199A and give it to a MSR at CapEd.
Payroll Deduction Form – If you would like your Direct Deposit split between different
accounts within the Credit Union, you will need to fill out a payroll deduction form. Otherwise,
Direct Deposits will default to your savings account.
Automatic Payment Request Form – If you have money automatically withdrawn from your
checking account each month (i.e., mortgage, utility billing, insurance) please fill out a form for
each company. We will help mail the forms to those companies to change the withdrawals to
your CapEd account. CapEd also offers an online bill pay program if you would prefer this
method for paying your bills.
Letter of Closure - This letter requests your previous financial institution to close your existing
account and transfer funds to your CapEd account. Please fill out the form and mail it to your
previous financial institution. Be sure all your direct deposits and automatic transfers are active
in your CapEd account before closing your other financial institution’s checking account.
