S.M.A.R.T. Goal Action Plan Template

S.M.A.R.T. Goal (Specific, Measureable, Attainable, Relevant, Time-based)

Example – We will decrease the percentage of bullying from 64% to 58% by the

end of the school year as evident by the Student Discipline Report.

The action plan should include two components.

 What do you need from the administration, organization or school

counselor?

 What will you do differently to decrease bullying at school?

Example – We would like an after-school workshop on anti-bullying and a few

examples of lesson for students in on assertive discipline or conflict resolution.

During the school year, our YAP organization will work with school principal, vice

principals, and school counselors to develop an anti-bullying campaign.
