Fair Credit Reporting Act Disclosure & Authorization for Volunteers
PLEASE SIGN AND RETURN TO THE YMCA
The YMCA of the Greater Houston Area conducts criminal history checks on prospective volunteers as part of the
final selection process. The employer’s right to check for a criminal background is governed by the Fair Credit
Reporting (“FCRA”). A report on a volunteer’s history is considered to be a “consumer report” obtained from a
consumer reporting agency and under these circumstances, you, the prospective volunteer, are considered to be
a “consumer.”
The YMCA of Greater Houston will obtain a “consumer report” about you. If the YMCA of the Greater Houston
Area considers any information in the “consumer report” when making a decision that directly and adversely
affects you, you will be provided with a copy of the “consumer report” before the decision is finalized. A copy will
also be forwarded to you by contacting the Human Resources Department. You may also contact the Federal
Trade Commission about your rights under the FCRA as a “consumer” with regard to “consumer reports” and
“consumer reporting agencies.”
Authorization
By signing below, I, _______________________________________, hereby voluntarily authorize the YMCA of
the Greater Houston Area to obtain “consumer reports” about me from a “consumer reporting agency” and to
consider the “consumer reports” when making decisions regarding volunteering at the YMCA of Greater Houston.

I understand that the policy of the YMCA is to refer all inquires from the media or press to the

Executive Director of the branch and that I may not speak for the YMCA.
I understand the policy of the YMCA is to cooperate with the authorities in the investigation of

suspected child abuse and molestation situations. I, as a volunteer, agree to cooperate with
the investigations as requested.
