Power of Attorney Information

In order to purchase a Fighting Cystic Fibrosis license plate, you have to visit your local Alabama County

Motor Vehicle Office to complete a commitment to purchase form. If you wish, you can fill out the

attached Power of Attorney form that will allow a Laps for CF representative to complete the form on

your behalf. Important information to note:

The Power of Attorney (POA) form must be filled out and signed by the person who owns the

vehicle (person whose name is on the car title). If this person owns more than one vehicle, you

can use the same form. If you family owns two vehicles but they are in two different names, you

must use two POA forms.

This form needs to be notarized. If you do not know a notary, you should be able to go to any

bank to get the form notarized.

Please mail the ORIGINAL SIGNED copy of the POA form to the Laps for CF office with your

check. We cannot accept a copied or scanned version of the POA.

You MUST include a check for $50 per license plate/vehicle with the form. Please see the

following page for information on who to make the checks payable to according to your county

of residence.

When a Laps for CF representative takes your form & check to the Motor Vehicle Office, we will

receive a receipt that we will mail back to you. YOU MUST HOLD ONTO THIS RECEIPT IN ORDER

TO GET YOUR CF LICENSE PLATE. THE STATE WILL NOT ALLOW YOU TO PICK UP YOUR PLATE

WITHOUT THIS RECEIPT. You will keep your current license plate until 1,000 CF plates are sold at

which time you will be notified that the CF plates are ready for pick up.
