Liability Insurance

I accept that it is my responsibility to ensure that my child has adequate liability insurance and insurance

for loss or damage to personal property and that coverage in the amount of $1.000,000 is required. I

understand that I will be required to provide proof of liability insurance coverage before my child is

allowed to participate in the activity/trip and that failure to provide such proof may result in my child being

denied participation without any right to compensation for losses due to deposits or non-refundable

amounts paid.

Medical Insurance

I accept that it is my responsibility to ensure that my child has adequate out of province medical coverage

in addition to student insurance. I agree that there may be costs associated with the recovery, transport and

treatment of our child if our child is injured while travelling or while engaging in the activity/trip and we

hereby agree to be responsible for and pay any and all such costs that may arise. I understand that I will be

required to provide proof of medical insurance coverage before my child is allowed to participate in the

activity/trip and that failure to provide such proof may result in my child being denied participation without

any right to compensation for losses due to deposits or non-refundable amounts paid.
