Only one copy of each document is required. However, you are advised to obtain and keep a duplicate copy of each

document for your records.

SECOND: Have your required medical examination

 10. MEDICAL EXAMINATION: Each applicant must have a medical examination at either the Australian Embassy

Clinic in Vientiane or the North Eastern Wattana Hospital in Udorn, Thailand. The forms and further information for

the medical examination are enclosed. The applicant must pay for the medical examination and any additional

necessary medical procedures.

 11. VACCINATIONS: It is required by law that each person going to live in the United States be vaccinated against

several diseases. If you did not previously receive these vaccinations, the doctor will provide them to you and will

give you a record showing that you received the vaccinations. This is a very important document that you will need

in the United States.

THIRD: Fill out all of the following forms. Answers to all questions should be written in English. All questions must be

answered. If the answer is none, write "NONE"; Do not write "N/A" or "Not Applicable."

If your children will apply with you, one copy of Form DS-156, DS-156/Lao, and DS-230 Part I must be completed for each

child. Also, bring the evidence that the child is your biological child – photos of you with your child at different age, etc…

FOURTH: As soon as you have obtained all of the documents, carefully read the statement below, sign and date it, and

return this checklist to the Consular Section so that we may schedule you for a visa interview. You should bring all the forms

listed above with you to the interview.
