Challenger Library and Information Center (CLIC) Summer Checkout

Parent/Guardian,

Every year, we invite current 6

and 7

grade students to check out books over the summer. Please carefully read over

and complete the following form and return it if your student would like to check out CMS library books over the

summer. Please indicate below the number of books you will allow your student to borrow (up to 8 maximum).

Important dates:

May 25-June 9: Students may check out books for the summer during this time period.

June 29: The CLIC will be open from 9 a.m.-2 p.m. for students to return and/or check out books

July 29: The CLIC will be open from 9 a.m. – 2 p.m. for students to return and/or check out books

August 17 : All summer checkouts are due

If for any reason you are unable to return and/or check out the book(s) directly to CMS or any other D20 school, you

may mail them to Challenger Middle School, 10215 Lexington Drive, Colorado Springs, CO 80920.

 I do not plan to disenroll my student from District 20 or move out of the area during the summer.

 I understand that that these books must be returned by August 17, 2016

 I understand that if they are not returned, a fine for each book will be entered into the Infinite Campus system

(if the student is currently enrolled), or Challenger Middle School will withhold my student’s record (if my

student leaves D20 and attempts to enroll in another school district).

 I understand that, should our situation change and we leave the district, I will return the book(s) directly to

Challenger or will mail them to the school at the address listed above.
