EQUIPMENT-HYDRAULIC LIFT GATE SERVICE ITEM 500
Where carrier is required or requested to employ mechanical loading or unloading devices, including
hydraulic lifting or lowering devices, to accomplish pickup and delivery of the goods to or from carriers
vehicle, an additional charge of $100.00 will apply.
The charges for this service shall be paid by the party for whom the service is performed or guaranteed by
the shipper. Carrier is not obligated to perform such service, when suitable vehicles, equipped with such
devices and operators are not available. Services will only be rendered at such locations as are safe and
accessible to the vehicle.
Item 525
EQUIPMENT ORDERED AND NOT USED-ITEM 525
When the carrier is requested to perform transportation services and dispatches equipment to a point
designated, in accordance with the instruction of the party ordering the equipment, and instruction to the
carrier is later changed and the equipment is not used through no fault of the carrier, a charge of $1.50 per
mile will be made from the point of dispatch, subject to a minimum charge of $250.00 per truckload or
container, and $125.00 for less than truckload shipments.
Item 550
INSIDE PICKUP / DRIVER ASSIST AND DELIVERY-ITEM 550
When requested by consignor or consignee and carrier’s operating conditions permit, the carrier may move
shipments or portions of shipments from or to positions beyond the immediate adjacent loading or
unloading positions; (Inside Pickup and/or Delivery).
Service under this item will be provided to floors above or below the level accessible to carrier’s vehicle
only when elevator or escalator service is available and labor, when necessary to operate same, is provided
without cost to the carrier.
Service provided under this item will be accessed a charge of $3.48 per 100 pounds subject to a minimum
charge of $38.60 per shipment and a maximum charge of $347.30.
Item 600
NOTIFICATION PRIOR TO DELIVERY-ITEM 600
A. When a Bill of Lading and/or any Shipping Order is tendered bearing any notation requesting carrier to
notify a consignee prior to delivery by any means the charge for this service shall be $10.00 per occurrence.
B. When a shipment is tendered with more than one stop and requires notification prior to delivery for any
portion, the $10.00 charge shall apply for each separate stop-off.
C. The services described in this item are provided as a requirement requested by the shipper and not at the
carrier’s discretion or convenience. Accordingly, a shipper’s Bill of lading notation that the request for a
call before delivery is for the “carrier convenience only’ shall be construed as a request for notification
prior to delivery, and notification charges will apply.
Item 700
PICKUP OR DELIVERY SERVICE-SAT, SUNDAY OR HOLIDAY-ITEM 700
Carrier does not obligate itself to provide pickup or delivery service on a Saturday, Sunday or holiday.
However, when a customer requests Carrier to pickup or deliver freight on a Saturday, Sunday or holiday,
and this service is agreed to by Carrier, shipments will be subject to the following charges in addition to all
other applicable charges:
A. $4.750 per 100 lbs. Subject to a minimum charge of $157.75 and a maximum charge of $525.00
B. When service exceeds 8 hours, the charge will be at the rate of $120.00 per hour or fraction
thereof.
