By signing below, I agree to the following:

I understand that I am borrowing one 32 GB WiFi iPad (iPad3) for the 2012-2013 school year. This iPad loan will

include a berry-colored case, power adapter and projector cable.

I agree to have students collect data with the iPad using the SatCam app. (satcam.ssec.wisc.edu/)

•

Ethical Device Usage: The primary purpose for the iPad is educational and as such I am free to download any

•

educationally appropriate app, movie or software using my personal iTunes or other account.

I am free to use the iPad for personal email and Internet but do so at my own risk.

•

I understand that it is my responsibility to make periodic backups of any content I want to keep.

•

I agree that the UW-Madison will not be responsible for lost or damaged work or for any personal content on

•

the device. Files saved to the hard drive will be erased and replaced with the standard preloaded files and apps

anytime the iPad is returned, whether it is the end of the school year or mid-year servicing.

I will not tamper with the hardware or remove components from the iPad.

I agree to contact the Schwerdtfeger Library immediately if the iPad is damaged or malfunctioning. I will ship the

•

iPad back at my own expense via expedited shipping (e.g. Fed Ex, UPS) for servicing. I will report the repair

condition prior to shipment. (library@ssec.wisc.edu)

I understand that the iPad has AppleCare protection for repairs but is otherwise uninsured and that I will assume

•

financial liability for the iPad and accessories while they are in my possession and agree to pay the replacement

cost of $772.00 if stolen, lost or damaged beyond repair.

I agree to ship the iPad and accessories back at my own expense by June 1

I understand that I will be billed $772 if I do not return the iPad and accessories by the June 1

2013 due date.
