CORRECT SPELLING OF NAME

ATTENTION COACH - Each athlete on your squad must be wearing the number on the uniform as printed in

the State Track and Field Tournaments Official Program. If there is an error in the number printed or it is

necessary to change a uniform number, this change must be properly reported on this form a minimum of 10

minutes prior to the competition in the event. This form may also be used to correct a misspelled name.

Please indicate a spelling correction by an *.

Present the completed and signed form to the Clerk of Course in the Reporting Area or to the Head

Event Judge in a field event.
